Responsibilities and Duties
• Conduct face-to-face, phone, or virtual sessions with adults to address mental health, substance use, academic/vocational goals, physical wellness, housing stability, food security, and safety.
• Collaborate with clients to develop tailored service plans.
• Ensure timely completion of all case-related documentation.
• Establish linkages to community supports, promoting personal responsibility, accountability, and independent living skills.
• Assist clients with domestic, legal, medical, housing, school, and benefit issues.
• Compile and submit monthly statistics to supervisors.
• Adhere strictly to all agency and regulatory policies and expectations.
• Maintain organized documentation in accordance with established timelines and standards.
Qualifications
REQUIRED
• BA, or experience in a mental health or human services related field.
• Excellent communication skills.
• Clean Valid Driver License
• Registered, insured vehicle in operating condition
• SEC Clearance
• Justice Center Clearance
PREFERRED
• Knowledge and understanding of substance use disorder, prevention, and risk factors
• Basic understanding of mental illnesses
• Basic understanding of chronic illnesses
• General knowledge of Ulster County’s human services agencies.
• Knowledge and sensitivity to vulnerable populations.
Hours and Pay Rate
This is a full-time, 40-hours per week position based at Family of Woodstock’s Kingston office at 31 Albany Ave.
The rate of pay for this position is: $18.13/hour and includes agency-funded health insurance and other benefits.
