The Long Term Case Management program provides rental support and intensive case management services for individuals and families who have a documented disability and are homeless at the time of entry into the program.

Program Hours: 9:00am to 5:00 pm, Monday through Friday. Program supervisory staff are available 24 hours a day through the county-wide hotline.
